BackgroundResponsive neurostimulation has been utilized as a treatment for intractable epilepsy. The RNS System delivers stimulation in response to detected abnormal activity, via leads covering the seizure foci, in response to detections of predefined epileptiform activity with the goal of decreasing seizure frequency and severity. While thalamic leads are often implanted in combination with cortical strip leads, implantation and stimulation with bilateral thalamic leads alone is less common, and the ability to detect electrographic seizures using RNS System thalamic leads is (...) uncertain.ObjectiveThe present study retrospectively evaluated fourteen patients with RNS System depth leads implanted in the thalamus, with or without concomitant implantation of cortical strip leads, to determine the ability to detect electrographic seizures in the thalamus. Detailed patient presentations and lead trajectories were reviewed alongside electroencephalographic analyses.ResultsAnterior nucleus thalamic leads, whether bilateral or unilateral and combined with a cortical strip lead, successfully detected and terminated epileptiform activity, as demonstrated by Cases 2 and 3. Similarly, bilateral centromedian thalamic leads or a combination of one centromedian thalamic alongside a cortical strip lead also demonstrated the ability to detect electrographic seizures as seen in Cases 6 and 9. Bilateral pulvinar leads likewise produced reliable seizure detection in Patient 14. Detections of electrographic seizures in thalamic nuclei did not appear to be affected by whether the patient was pediatric or adult at the time of RNS System implantation. Sole thalamic leads paralleled the combination of thalamic and cortical strip leads in terms of preventing the propagation of electrographic seizures.ConclusionThalamic nuclei present a promising target for detection and stimulation via the RNS System for seizures with multifocal or generalized onsets. These areas provide a modifiable, reversible therapeutic option for patients who are not candidates for surgical resection or ablation. (shrink)

What is living well? We describe two contrasting lives and ask whether one is better lived than the other. Many philosophers, among them Susan Wolf, Richard Kraut and Stephen Darwall would say so. We criticize their position, which views certain activities as intrinsically more worthy than others. Instead, we conclude that persons are living well if they act morally and find long-term satisfaction, regardless of the pursuits they choose.

In the course of writing a book on Free Will, I took the opportunity to read a good deal of contemporary literature on the Free Will problem. This paper is a survey and reflection on that reading, responding to the current trends and state of play concerning the existence of free will.

American Academy of Pediatrics (AAP) and American College of Medical Genetics (ACMG) recently provided two recommendations about predictive genetic testing of children. The Clinical Sequencing Exploratory Research Consortium's Pediatrics Working Group compared these recommendations, focusing on operational and ethical issues specific to decision making for children. Content analysis of the statements addresses two issues: (1) how these recommendations characterize and analyze locus of decision making, as well as the risks and benefits of testing, and (2) whether the guidelines conflict or (...) come to different but compatible conclusions because they consider different testing scenarios. These statements differ in ethically significant ways. AAP/ACMG analyzes risks and benefits using best interests of the child and recommends that, absent ameliorative interventions available during childhood, clinicians should generally decline to order testing. Parents authorize focused tests. ACMG analyzes risks and benefits using the interests of the child and other family members and recommends that sequencing results be examined for additional variants that can lead to ameliorative interventions, regardless of age, which laboratories should report to clinicians who should contextualize the results. Parents must accept additional analysis. The ethical arguments in these statements appear to be in tension with each other. (shrink)

Large-scale sequencing tests, including whole-exome and whole-genome sequencing, are rapidly moving into clinical use. Sequencing is already being used clinically to identify therapeutic opportunities for cancer patients who have run out of conventional treatment options, to help diagnose children with puzzling neurodevelopmental conditions, and to clarify appropriate drug choices and dosing in individuals. To evaluate and support clinical applications of these technologies, the National Human Genome Research Institute and National Cancer Institute have funded studies on clinical and research sequencing under (...) the Clinical Sequencing Exploratory Research program as well as studies on return of results. Most of these studies use sequencing in real-world clinical settings and collect data on both the application of sequencing and the impact of receiving genomic findings on study participants. It is suggested that much of what passes for recursion in this field is in fact not recursive all the way down. After the paradoxical meaning of radical recursion is adumbrated, topology is employed to provide some examples. The properties of the Moebius strip prove helpful in bringing out the dialectical nature of radical recursion. (...) The Moebius is employed to explore the recursive interplay of terms that are classically regarded as binary opposites: identity and difference, object and subject, continuity and discontinuity, etc. To realize radical recursion in an even more concrete manner, a higher-dimensional counterpart of the Moebius strip is utilized, namely, the Klein bottle. Increasing university students’ engagement with ethics is becoming a prominent call to action for higher education institutions, particularly professional schools like business and engineering. This paper provides an examination of student attitudes regarding ethics and their perceptions of ethics coverage in the curriculum at one institution. A particular focus is the comparison between results in the business college, which has incorporated ethics in the curriculum and has been involved in ethics education for a longer period, with the engineering college, which (...) is in the nascent stages of developing ethics education in its courses. Results show that student attitudes and perceptions are related to the curriculum. In the broadest sense, Propositionalism is the view that knowing how to do something first requires our possessing appropriate propositional attitudes about an action. Non-propositionalism concerning knowing how, is the rejection of propositionalism. This distinction, and the rejection of propositionalism is typically traced back to Gilbert Ryle. In the 21st century, propositionalists have tried to turn the tables with a quick and decisive argument against non-propositionalism. According to the argument from pervasive inability, since (1) There are numerous cases in which (...) someone knows how to do something while possessing a pervasive inability to perform the action she knows how to do, and (2) these cases can be unproblematically explained to be instances of knowing how by propositionalism, but (3) these cases cannot be explained to be instances of knowing how by non-propositionalism, therefore (4) non-propositionalism must be rejected in favor of propositionalism. In this paper, through a careful examination of proposed cases, I argue that premise two is not true, and by correcting propositionalists’ mischaracterization of non-propositionalism, I argue that premise three is also not true. As a result, the argument from pervasive inability fails to provide a decision between propositionalism and non-propositionalism concerning knowing how. (shrink)

In the decades since Robert Nozick posed his now famous thought experiment involving the experience machine, philosophers have taken his treatment as conclusive. A review of the literature finds almost no one who has argued that people would choose the experience machine. To find such unanunity among philosophers is unexpected. But the situation is especially surprising because Nozick's conclusion appears mistaken. In support of this view, we offer three different sorts of reasons why persons would be inclined to choose the (...) experience machine. In recent years there has been an increased interest in the application of Aristotelian virtue to business ethics. The objective of this paper is to describe the moral and intellectual virtues defined by Aristotle and the types of pedagogy that might be used to integrate virtue ethics into the business curriculum. Virtues are acquired human qualities, the excellences of character, which enable a person to achieve the good life. In business, the virtues facilitate successful cooperation and enable the community to (...) achieve its collective goals. The cultivation of virtue in students requires imparting knowledge about virtue and training students to be virtuous. A variety of instructional techniques are discussed including using case studies, collaborative and cooperative learning, role-playing, and video presentations. Business educators should emphasize to students that virtue considerations apply both to possible actions they may take and to themselves as moral agents. Since faculty may be viewed as role models, it is especially important that they set proper standards of behavior for students to emulate. (shrink)
